Corect ar fi așa:

I could not understand the questions.

Varianta can't (adică forma scurtată pentru cannot) este la timpul prezent, iar acțiunea este la timpul trecut. Cu alte cuvinte nu poți acum înțelege întrebările, câtă vreme examenul a trecut deja. Ai înțeles ?
